7/30/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Ben-nita. Upon arrival to the home setting, Ben-nita appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about any new behaviors since the last session. Ben-nita verbalized to QMHP-C she got into an argument with her mother because her mother set her cousin up and that was âfoulâ. 2. Behavior: Ben-nita participated in the session and did not need any redirection from QMHP-C. Ben-nita presented normal behaviors; however, at times she got hostile when discussing different topics. 1. Intervention: QMHP-C began the session by inquiring about Ben-nitaâs current thoughts, feelings and emotions. QMHP-C provided redirection for incorrect or inappropriate thought patterns regarding ability to interact positively with others. QMHP-C provided interventions and strategies to address importance of accepting mistakes as a learning tool for growth instead of continuing to blame others, becoming depressed, anxious or angry, or minimize own behaviors. QMHP-C processed examples of times when Ben-nita made mistakes to include who was involved, how it made them feel, how they responded to those feelings, and how it affected them after the mistake occurred. 2. Intervention: QMHP-C- utilized role play to provide alternate methods of response in order to assist Ben-nita in understanding that they did not have to be overtaken by strong emotions and that it was possible to make a mistake, accept responsibility for it, apologize and find a positive solution for that mistake instead of becoming verbally / physically aggressive, withdrawing, or avoiding in the future. QMHP-C reemphasized importance of relaxation and calming techniques and explored Ben-nita ability to de-escalate given certain scenarios. 1. Response: Ben-nita appeared to be receptive to activities and interventions saying she hadnât really considered how thoughts or feelings toward concepts of respect, courage, responsibility, etc would affect daily living and perception. She demonstrated ability to remember and recite skills discussed and how she attempted to apply those skills over throughout the week. 2. Response: Ben-nita reports being excited about learning new things about herself in a way that isnât degrading or stressful as it gives her an opportunity to know who she really is instead of who people say she is in comparison to other family members.
